CYSHCN Contractors

Maternal and Child Health's Title V contracts with community-based organizations in select counties. Community-based organizations provide case management and support services to children with special health care needs (CSHCN) and their families.

There are two kinds of funded activities: 

• Case Management.
• Family Supports Community Resources.
