Obtaining a New Interventional Breast Radiography Certification
If a facility has an interventional breast unit(s) used for breast biopsy and/or needle localizations, the facility is required to have an Interventional Breast Radiography Certification. Units that are used for both mammography and interventional breast radiography will be included on the mammography certification.
- Complete and submit to the Mammography Certification Program the following forms with the appropriate fee within 30 days of beginning use of interventional breast equipment:

Title 25 of the Texas Administrative Code (TAC) §289.230(gg)(14) requires each registrant to have and implement written operating and safety procedures.
Note: If the facility plans to perform Mobile Interventional Breast Radiography complete section 2 of the application and submit the requested documents and procedures.
Fee Schedule
Each application for a New Interventional Breast Radiography Certification shall be accompanied by a one-year non-refundable fee. An invoice will be mailed annually for the interventional breast radiography certification fee. Fee payments by check or money order shall be made payable to Texas Department of State Health Services

Annual Fees for Mammography Certification
Annual fees must be paid each year in order to continue operating mammography devices. The fee covers all direct and indirect costs associated with licensing a company or individual.

Interventional Breast Radiography Certification Renewals
Interventional breast radiography renewal documents shall be submitted every three years for renewal.
The agency will send a renewal reminder letter and the application by mail 2-3 months prior to the expiration date of the mammography certification. Items requested for renewal are:

Name and/or Ownership Change

To Change a Mailing and/or Physical Use Location Address(s)
Complete the appropriate sections of RC 230-04,Interventional Breast Radiography Application Amendment (PDF, 77KB) and submit to the Mammography Certification Program.
Interventional Breast Radiography Equipment Changes
To add an interventional breast radiography unit submit:
- RC 230-04,Interventional Breast Radiography Application Amendment
- Copy of the current medical physicist’s survey report for the new unit
To delete an interventional breast radiography unit complete and submit RC 18-M, Mammography Transfer-Disposal Form (PDF, 17KB).
Personnel Changes
Notification of additions or deletions in personnel is no longer required for interventional breast radiography certifications, with the exception of the radiation safety officer.

Mobile Services Authorization
Mobile services authorization must be obtained prior to providing mobile interventional breast radiography services.
To add mobile services authorization to the interventional breast radiography certification, submit:
- RC 230-04 Interventional Breast Radiography Application Amendment
- Required documents and procedures listed in the mobile service authorization section of the application
To remove mobile services authorization
- Notify the agency in writing by submitting a signed letter requesting removal of Mobile Service Authorization
- Include the facility’s mammography certification number
