The School Coverage webpage includes data from the DSHS Annual Report of Immunization Status. All public school districts and accredited private schools in Texas must complete the Annual Report of Immunization Status each year. The annual report provides immunization coverage levels for required vaccines among students in kindergarten and seventh grade, as reported by the school/district. It also captures data on the number of filed conscientious and medical exemptions. It is important to note that homeschooled children or children attending non-accredited private schools are not included in this report.
The conscientious exemption levels reported here represent the exemption levels among children attending public and private schools in Texas. Because children attending home schools are not included in this report, these levels do not represent exemption rates for the state of Texas or its public health regions. Online data is maintained for five years. More historical data is available upon request. Please complete requests using the data request form.
In accordance with Texas Administrative Code §97.62, Texas law allows for an exemption from immunizations for reasons of conscience, including religious belief. To claim an exemption due to reasons of conscience, a student's parent or legal guardian must request, sign, and submit an official DSHS affidavit form to the child's school. The affidavit is valid for two years from the notarization date.
Note: Texas Administrative Code §97.62 states that students who haven't received the required immunizations for reasons of conscience may be excluded from school in times of emergency or epidemic declared by the Texas Commissioner of Health.

This survey is conducted annually to validate the results of the DSHS Annual Report of Immunization Status in Texas Schools. A random sample of public and private schools is selected for the survey each year. Local health department and DSHS staff review a random sample of immunization records from each selected school. Statewide vaccination coverage levels are available for kindergarten and seventh grade students attending public and private schools in Texas.
